The Estradiol Test That Reads Wrong in Men

Someone gets bloodwork done mid-cycle. Estradiol comes back at 65 pg/mL, well above the range printed next to it. They take an aromatase inhibitor. A week later their joints ache, libido is gone, and they feel flat — so they conclude the estradiol was even higher than they thought and take more.
The number was probably wrong to begin with.
Two different tests carry the same name
Laboratories measure estradiol in two fundamentally different ways.
The immunoassay is the default almost everywhere. It is fast, cheap, and was developed for the levels found in women, which are ten to a hundred times higher than in men. It works well there.
In men it does not. At the low concentrations found in male blood, the immunoassay cross-reacts with other steroids circulating in the sample — and on cycle there are a great many of those. The result is a reading that is too high, sometimes substantially.
The sensitive assay uses mass spectrometry — usually written as LC-MS/MS. It separates the molecules physically before measuring them, so nothing else in the sample gets counted as estradiol. This is the method that produces a number worth acting on in men.
Why it matters more on cycle than off
Cross-reactivity depends on what else is floating around. Someone running several grams of assorted androgens has far more structurally similar compounds in their blood than an untreated man — exactly the situation where the immunoassay drifts furthest, and exactly the situation where people are most likely to be testing.
The test is least reliable in the population that relies on it most.
What to ask for
On the lab request, the wording that matters is "estradiol, sensitive" or "estradiol by LC-MS/MS". Some labs list it as ultrasensitive. If your report does not state the method, it was almost certainly the standard immunoassay.
It costs somewhat more. Set against the cost of crashing your estradiol and spending six weeks getting it back, that is not a close comparison.
Crashed estradiol is worse than high estradiol
This is the part that gets underestimated, because the symptoms are less dramatic on the way down.
High estradiol announces itself: water retention, puffiness, sensitive nipples, mood swings. Unpleasant, visible, and it motivates action.
Low estradiol arrives quietly. Joints that ache and feel dry. Libido gone — not reduced, gone. Flat mood, poor sleep, no motivation. And here is the trap: those symptoms overlap almost completely with estradiol being too high. Someone who does not know their real number, feels bad, and assumes the aromatase inhibitor is not working yet, takes more — and drives themselves further into the state causing the problem.
Estradiol is not a hormone to be minimised. It maintains bone in men, protects the joints, drives libido, and supports the lipid profile that androgens are already damaging. The target is a healthy range, not the lowest achievable figure.
The order that works
- Sensitive assay first. No aromatase inhibitor on the basis of a standard test.
- Symptoms alongside the number, not instead of it. Both point the same way only when you know which direction you are in.
- Lower the dose before adding a drug. Estradiol rises because aromatisable androgens are present in quantity. That is the cause; the aromatase inhibitor treats the consequence.
- One change, then wait. Anastrozole has a half-life around two days, which means the level keeps climbing for a week and a half. Redosing after three days because nothing happened yet is how the accumulated total lands at once.
When an aromatase inhibitor cannot help at all
Worth knowing before you reach for one: if the compound does not aromatise, there is nothing to inhibit.
Trenbolone does not aromatise. Nandrolone barely does but is progestogenic. Symptoms on those compounds that look estrogenic — nipple sensitivity, lost libido, water — are usually driven by prolactin instead. Adding an aromatase inhibitor there makes things worse by lowering what little estradiol is present, and leaves the actual cause untouched.
That distinction is made with a prolactin test, not by how it feels.
The short version: before you act on an estradiol number, check which test produced it. It is the cheapest correction available in this whole field, and it prevents the most common self-inflicted problem in it.
