Benzoyl peroxide

Topical agent that kills the bacteria involved in acne without them developing resistance — the practical first line for back and shoulder acne under androgens.

It is an oxidising agent and will discolour fabric permanently. White towels and an old shirt, or let it rinse off fully as a wash.

Overview

Benzoyl peroxide is the least glamorous entry in this section and the one most worth having. It is cheap, available without prescription, and it works on exactly the acne that androgens produce — on the back and shoulders, where it appears first and is hardest to reach.

Mechanism of Action

It releases oxygen into the follicle, which kills Cutibacterium acnes, and it loosens the plug of dead cells blocking the pore. The important property is that bacteria do not develop resistance to it — unlike topical antibiotics, which lose effectiveness over months. That is why dermatologists pair the two rather than using an antibiotic alone.

Typical Context

The first thing to try when acne appears under a cycle, before anything systemic is considered. It does nothing about the androgen load driving the problem, but it manages the consequence at low cost and low risk.

For back acne specifically, use it as a wash rather than a leave-on gel: you can actually reach your own back with it, and a large area treated with a leave-on product gets irritated fast.

What the strength is for

More is not better. Trials comparing 2.5%, 5% and 10% find the same effectiveness — and steadily more irritation as the strength goes up. Starting at 2.5% and going up only if needed is the sensible order, not the timid one.

Harm-Reduction Notes

  • Start every other day and build up; the urge to attack it daily at full strength is what causes most of the irritation people quit over
  • Leave contact time short as a wash — a minute or two in the shower is enough
  • White towels, and let it rinse off properly before you touch good clothes
  • It treats the consequence. If the acne is severe and scarring, that is a conversation about the dose, the compound, or a dermatologist — not about a stronger gel
