The effect on platelets is irreversible and lasts the platelet's lifespan, roughly 7–10 days. That means cuts bleed longer, bruising increases, and gastrointestinal bleeding risk rises — particularly with alcohol or NSAIDs like ibuprofen. Even at low doses, aspirin can cause gastric irritation and ulceration. Taking it with food helps; a proton pump inhibitor is used where there is a history of ulcers. Black stools or persistent stomach pain mean stop and seek medical advice.
Overview
Low-dose aspirin appears in this context for a specific reason: androgens raise hematocrit, thick blood carries a clot risk, and aspirin is the accessible over-the-counter response to clot risk.
That reasoning is half right, and the half that is wrong matters.
Mechanism of Action
Aspirin irreversibly inhibits COX-1 in platelets, blocking thromboxane A2 production. Thromboxane is the signal platelets use to aggregate, so blocking it makes them less able to form a clot.
The irreversibility is the important detail. Platelets have no nucleus and cannot make new enzyme, so a single dose disables the affected platelets for their entire remaining lifespan — seven to ten days. This is why 75–100 mg daily is enough, and why the effect persists long after the drug has cleared.
The Distinction That Decides Everything
Aspirin affects clotting. It does not affect blood viscosity.
Elevated hematocrit is a viscosity problem: more red cells per volume means thicker blood, more resistance, higher blood pressure and a greater tendency to clot. Aspirin addresses the last item on that list and none of the others.
Someone at 56 % hematocrit taking aspirin still has 56 % hematocrit. The blood is still thick, the heart is still working against higher resistance, and the underlying cause — the androgen dose, dehydration, sleep apnoea — is untouched.
What actually lowers hematocrit:
- Donating blood or therapeutic phlebotomy — the direct and fastest lever
- Reducing the androgen dose — the cause
- Adequate hydration — a diuretic does the opposite
- Treating sleep apnoea if present — a frequently missed contributor
Aspirin belongs alongside those, not instead of them.
The Accidental Blood Thinner Stack
Worth naming because it assembles itself quietly. Nattokinase, omega-3 at high doses, curcumin with piperine and OPC all have mild antiplatelet activity. Each is individually trivial and each is commonly taken.
Add aspirin and the effects are additive. The result is a meaningful bleeding risk built out of things nobody thought of as medication. Before surgery, dental work or if bruising becomes noticeable, the whole list is worth reviewing rather than just the aspirin.
