Overview
Cabergoline is a dopamine agonist approved for elevated prolactin levels and, in higher doses, for Parkinson's disease. In the bodybuilding context, it appears almost exclusively in connection with 19-nor compounds such as nandrolone and trenbolone, whose progestogenic activity raises prolactin.
It is effective at what it does. But it is a genuine medication with a genuine side-effect profile, and it is regularly taken preventively without anyone having measured whether prolactin is elevated at all.
Mechanism of Action
Prolactin is released by the pituitary gland and inhibited by dopamine. Cabergoline binds to dopamine D2 receptors in the pituitary and thereby suppresses prolactin secretion — it mimics the body's own brake.
With a half-life of sixty to seventy hours, it is unusually long-acting, which is why a dose once or twice a week is sufficient. That length also means adjustments take days to take effect and that an overshoot corrects itself only slowly.
The reason it works against nandrolone and trenbolone side effects lies in those substances themselves: their progestogenic activity raises prolactin, which in turn causes loss of libido, erectile problems and reactions in breast tissue. These symptoms look estrogenic, which is why they are so often treated with an aromatase inhibitor — a substance that addresses a completely different mechanism and, by lowering estradiol further, tends to make matters worse.

Doses in this context sit far below those used in Parkinson's therapy, which is relevant for assessing risk: the cardiac valve changes documented in the literature relate to long-term treatment at several milligrams a day, not to a quarter milligram twice a week. That does not make the risk zero, but it does place it in proportion.
One side effect deserves separate mention because it is so rarely anticipated. Dopamine agonists can trigger impulse control disorders — gambling, compulsive shopping, hypersexuality. This is well documented in Parkinson's patients and is not confined to high doses. What makes it difficult is that the person affected typically does not recognise the change as a drug effect.
