Retatrutide
Also known as: Reta, Retatrutid, LY3437943, Triple Agonist, Triple G
Triple agonist with the strongest weight-loss results measured so far — and still in clinical trials, which means every use outside a study relies on grey-market material with no safety data.
Substance family
Gut hormone analogues that suppress appetite. Highly effective for fat loss, with the consistent caveat that a substantial share of the weight lost is lean mass unless protein and training compensate.

Dosing & protocols by goal
Rough orientation ranges as reported in harm-reduction literature — not an individual treatment plan. Doses are individual and open-ended; frequency depends on the ester or half-life.

Side effect & countermeasure
Typical problems and what you can do to reduce harm — at a glance. Does not replace medical monitoring.
Problem
No approval, no safety data, unverified grey-market material
Countermeasure
This is the defining risk and it cannot be dosed away. Retatrutide is only available as a research chemical, meaning content, purity and sterility are unverified. Only sources with an independent HPLC/MS analysis; even then, the absence of long-term human safety data remains.
Problem
Pronounced loss of lean mass
Countermeasure
The strongest weight loss in this class means the largest absolute muscle loss if training and protein do not compensate. Protein at roughly 2–2.5 g per kg of body weight, resistance training throughout, moderate deficit. Track body composition, not scale weight.
Problem
Elevated heart rate
Countermeasure
The glucagon component raises energy expenditure and, with it, resting heart rate — more so than with GLP-1 alone. Measure resting heart rate regularly; a persistent marked increase is a reason to reduce the dose or stop.
Problem
Nausea, vomiting, gastrointestinal side effects
Countermeasure
More pronounced than with semaglutide or tirzepatide at comparable steps. TitrateTitrateAdjusting a dose stepwise against a measured result rather than setting it in advance. slowly, start at the lowest dose, hold each step at least four weeks.
Problem
Effects on glucose metabolism through the glucagon component
Countermeasure
Glucagon raises blood glucose while GLP-1 lowers it — the net effect varies between individuals. Anyone with diabetes or insulin resistanceinsulin resistanceHow strongly the body responds to its own insulin — worsened by growth hormone and by some blood-pressure drugs. should not manage this without medical supervision. Monitor fasting glucose and HbA1c.
Problem
Dehydration and electrolyte shifts
Countermeasure
Reduced intake means less fluid and fewer electrolytes. Drink deliberately, keep sodium and potassium adequate.
Overview
Retatrutide is the current end point of a development line: where semaglutide acts on one receptor and tirzepatide on two, retatrutide acts on three. In the phase 2 trial it produced average weight loss of roughly twenty-four percent over forty-eight weeks — figures previously seen only after bariatric surgery.
The decisive point about this substance is not its effect but its status. Retatrutide is not approved anywhere in the world. It is still in clinical development, which means everything sold under this name outside a trial is research-grade material of unverified origin.
Mechanism of Action
Retatrutide activates the GLP-1 and GIP receptors, as tirzepatide does, and adds a third: the glucagon receptor.
Glucagon is usually thought of as insulin's counterpart, raising blood glucose. But it also increases energy expenditure and promotes fat breakdown in the liver. This third component is where the additional effect comes from — it does not just reduce intake but raises consumption.
Two consequences follow. The increase in energy expenditure also raises resting heart rate, more noticeably than with the other two substances. And the effect on glucose metabolism becomes harder to predict, because glucagon pushes blood glucose in the opposite direction to GLP-1. The net effect differs between individuals, which is one reason the substance requires more careful monitoring than its predecessors.
Typical Context
Retatrutide is used in diet phases where the previous generations were not sufficient or were poorly tolerated. Doses in practice generally stay well below the trial maximum of twelve milligrams — one to four milligrams a week is the usual range.
The honest assessment has to name the trade-off clearly. The effect is real and documented in trials of good quality. What is missing is everything that comes after approval: long-term safety data, established handling of side effects, and above all a product whose contents are guaranteed. A vial from the grey market with an unclear fill has an unknown actual dose, no matter how carefully the intended dose was calculated.
The same problem as with the whole class applies to muscle. The stronger the weight loss, the larger the absolute amount of lean mass at risk — and retatrutide produces the strongest weight loss in this group.
Side Effects & Risks
- No approval, no long-term safety data in humans
- Unverified content and purity of grey-market material
- Pronounced loss of lean mass with the large deficit
- Elevated resting heart rate through the glucagon component
- Nausea, vomiting, gastrointestinal side effects, more pronounced than with the predecessors
- Unpredictable effects on glucose metabolism
- Dehydration and electrolyte shifts
- Reduced training performance
Blood Work & Monitoring
- Resting heart rate — measured daily; the marker most specific to this substance
- Fasting glucose and HbA1c — the glucagon component makes the effect harder to predict
- Body composition — scale weight hides muscle loss
- Electrolytes (sodium, potassium)
- Lipid profile (LDL, HDL, triglycerides)
- Liver values (ALT, AST, GGT)
Harm-Reduction Notes
- The unverified source is the central risk — insist on an independent HPLC/MS analysis, and understand that it does not replace missing safety data
- Start at the lowest dose and titrate slowly; side effects are more pronounced than with semaglutide or tirzepatide
- Monitor resting heart rate; a persistent increase is a reason to reduce the dose
- Protein has to be planned deliberately — roughly 2–2.5 g per kg of body weight
- Keep resistance training running throughout
- Not for anyone with diabetes or insulin resistance without medical supervision
- Reconstitute sterile, store refrigerated, observe shelf life
Legal status by region
Regulation differs considerably between jurisdictions and changes over time. This is a rough orientation, not legal advice — check the rules that apply where you are.
| Region | Regulatory status |
|---|---|
| United States | Not approved for human useSold as a research chemical; marketing for human consumption is unlawful. |
| EU / UK | No marketing authorisationNeither a medicine nor a supplement — no legal route for human use. |
| Most other jurisdictions | Unregulated grey areaPossession is legal in many places precisely because no framework covers it. That is not an indication of safety. |
| Competitive sport | ProhibitedListed by WADA where a relevant class applies. |
