Mechanism of Action
Growth hormone acts partly directly and partly through IGF-1, which the liver produces in response. IGF-1 mediates most of the anabolic effects, which is why it is the marker measured in practice — GH itself is released in pulses and fluctuates too much to be meaningful in a single blood draw.
The metabolic effect runs in two directions. GH promotes lipolysis, the breakdown of fat, and simultaneously reduces the effect of insulin on cells. That second part is not a side effect at the margin — it is the central problem of prolonged use.
Insulin and growth hormone are functional opposites. Sustained elevated GH means the body needs more insulin for the same effect on blood glucose. Above that range, the effects that are visible in some competitive physiques appear — organ growth, including of the intestines. Those changes are not reversible in the way water retention is.
