OPC is sold with claims about ageing, skin, immunity and vision that rest largely on test-tube antioxidant capacity rather than clinical outcomes. The one endpoint with reasonable human data is blood pressure and vascular function. Everything else should be treated as unproven.

The OPC content of grape seed extracts differs substantially between products, and Pycnogenol (pine bark) is a related but distinct extract studied separately. A dose figure without a standardisation percentage says very little.
Overview
OPC — oligomeric proanthocyanidins — are the polyphenols that give grape seeds, pine bark and red wine their character. They are sold heavily on antioxidant claims, and the honest position is that one of those claims holds up reasonably well and the rest do not.
Mechanism of Action
The best-supported effect is on the endothelium, the inner lining of blood vessels. OPC increases the availability of nitric oxide there, so vessels relax and blood pressure falls slightly. That is the same broad pathway citrulline works through, arrived at differently.
The antioxidant activity itself is real in the sense that these are potent radical scavengers in a test tube. The question is whether that translates into anything measurable in a person, and for most claimed endpoints it has not been demonstrated. High antioxidant capacity in vitro is a poor predictor of clinical benefit — the history of antioxidant supplements is mostly a history of that gap.
Where It Fits Here
Blood pressure, as a small contributor. Meta-analyses of controlled trials find a few mmHg systolic reduction, most pronounced in people who started above normal.
That is worth having in context: elevated blood pressure is the most reliably damaged marker on cycle, and it is driven by water retention, rising hematocrit and vascular effects. A few mmHg from a polyphenol is a supporting measure alongside cardio, sodium control and keeping hematocrit in range. It is not a treatment, and readings that stay high need telmisartan, amlodipine or lisinopril.
Note also the accumulation: OPC, citrulline and tadalafil all lower blood pressure a little, and all three mildly affect platelets alongside omega-3 and nattokinase. Individually trivial, collectively worth measuring rather than assuming.
An Honest Placement
Among the supplements on this site, OPC sits below omega-3, creatine and glycine in terms of demonstrated value, and roughly alongside citrulline for the vascular endpoint.
It is cheap and safe, so it is a defensible addition for someone already covering the basics. It is not a reason to skip anything that matters more, and the anti-ageing framing it is usually sold under has essentially no support.
