Oxandrolone (Anavar)

Mild oral DHT derivative with genuine medical approval, well tolerated by the liver for a 17-alpha-alkylated compound — but harder on the lipid profile than almost anything else.

Severe drop in HDL — more pronounced than with most injectables

Countermeasure

The combination of oral administration and DHT structure hits HDL particularly hard. Cardio as the base, plus omega-3 and citrus bergamot; with strongly deteriorated values, ezetimibe or pitavastatin under medical guidance. Measure a baseline before starting — without it, later values cannot be judged. If a statin is started, CoQ10 belongs alongside it — statins deplete it through the same pathway.

Countermeasure

Mild for an oral compound, but not absent. Check ALT, AST and GGT before and during use; avoid alcohol; do not combine several orals. Limit duration to 6–8 weeks rather than extending it. TUDCA at 500–1000 mg daily addresses the cholestatic mechanism directly; NAC supports the antioxidant system alongside it.

Oxandrolone suppresses the axis reliably, including at low doses. A post-cycle plan is necessary here too — being well tolerated is not the same as being harmless to the hormonal system.

Oxandrolone does not aromatize. Used alone, estradiol drops along with natural testosterone — do not add an aromatase inhibitor on top.

Overview

Oxandrolone, almost always referred to as Anavar, is one of the few anabolic steroids with a current human medical indication. It is approved for weight regain after severe burns, major surgery and chronic wasting conditions, and has been used in children in that context — an application that says a great deal about its tolerability at therapeutic doses.

Its reputation in the bodybuilding scene follows from this: Anavar is regarded as the mild oral. That is accurate for the liver and largely accurate for androgenic side effects. It is emphatically not accurate for the lipid profile, which oxandrolone affects more severely than most injectable steroids.

Mechanism of Action

Oxandrolone is a DHT derivative with two modifications. An oxygen atom replaces a carbon in the A-ring, which makes it resistant to breakdown by 3-alpha-hydroxysteroid dehydrogenase and thereby prolongs its action in muscle tissue. The 17-alpha-alkylation makes it orally available by protecting it from being broken down on first pass through the liver.

That second modification is the reason every oral steroid carries liver relevance — the same property that lets the substance survive the liver also means the liver has to process it. Oxandrolone is at the mild end of this class, but "mild" describes a position on a scale, not an absence.

The effect on the lipid profile stems from both properties combined. Oral administration means the substance reaches the liver in high concentration, where it alters the activity of hepatic lipase; the DHT structure amplifies the effect. The result is a drop in HDL that is regularly more pronounced than with injected compounds at comparable dosing.

Typical Context

Oxandrolone is used in definition phases, over six to eight weeks, and generally alongside a testosterone base. Because of its short half-life, the daily dose is usually split into two administrations.

It is also one of the few compounds regularly used by women, since virilising side effects occur less readily than with other substances. Less readily does not mean not at all — voice changes in particular are not reversible once they occur, which is why doses in that context sit far lower and monitoring matters more.

The price is a practical problem in its own right. Oxandrolone is expensive to synthesise, which makes it a frequent target for counterfeiting. Analyses of black-market samples repeatedly find methandienone or stanozolol in tablets sold as Anavar. Anyone experiencing marked water retention or rapid weight gain is almost certainly not taking oxandrolone.

Harm-Reduction Notes

  • Measure a lipid baseline before starting; without it the extent of the change cannot be assessed
  • HDL recovers slowly — plan a genuine break rather than chaining cycles
  • Do not combine with other oral compounds; the liver burden is additive
  • Limit duration to 6–8 weeks
  • Avoid alcohol during use
  • Treat the counterfeit risk seriously: water retention does not fit oxandrolone and indicates a different substance
  • Being well tolerated does not mean the axis is spared — a post-cycle plan is necessary
